Suction Assisted Laryngoscopy and Airway Decontami

Suction Assisted Laryngoscopy and Airway Decontamination (SALAD) technique:

1. Begin with optimal patient positioning for endotracheal intubation (ETI).
2. Hold the rigid suction catheter (RSC) in the right hand, mirroring the curve of the upper airway structures.
3. Insert the RSC into the patient’s mouth and continuously suction as it is advanced just ahead of the laryngoscope blade around the base of the tongue.
4. Grip the RSC tightly, using it to displace the tongue and lower jaw to maximize space for laryngoscopy and tube delivery.
5. Ensure continuous suction to clear the airway and keep the camera lens clean if video laryngoscopy is employed.
6. The RSC primarily displaces the upper airway structures until the laryngoscope blade is optimally positioned.
7. Use the RSC to suction the glottis and proximal trachea under direct visualization.
8. Withdraw the RSC and re-insert it into the mouth to the left of the laryngoscope while maintaining visualization of the proximal esophagus and glottic opening. This is known as the SALAD Park Maneuver.
9. “Park” the catheter in the upper esophagus position, allowing continuous suctioning of the hypopharynx, even if the patient continues to vomit or bleed.
10. With the right hand freed, deliver the endotracheal tube while optimizing space and maintaining good visualization.
11. Once the endotracheal tube is positioned between the vocal cords, suction the lumen of the tube with a soft catheter to clear any residual contaminants prior to ventilation. #SALAD #suction #AIROsuction

Resuscitation of a combat casualty would almost certainly involve the administration of warmed whole blood within the first 30 mins of injury to limit the dose of shock and subsequent oxygen debt before becoming irreversible.
